For more than 100 years, Illinois Masons have taken up the charge to support and provide healthcare services to our fellow Illinois Freemasonry Family, who reside in Illinois. During the last 20 years, the Masonic Family Health Foundation (MFHF), in collaboration with the Advocate Illinois Masonic Medical Center (AIMMC), has stayed true to its legacy of providing these valuable healthcare services. To provide direct healthcare services to Illinois Masons and their families, the MFHF’s Masonic Relations Committee created the Masonic Assistance Program (MAP). This program was developed and designed specifically to help cover costs for medical and dental services for Illinois Master Masons and their families (spouse, domestic partner, dependents, or widow) who qualify and are accepted into the program.
A Master Mason applying to MAP for assistance must be a member in good standing of a Masonic Lodge in the State of Illinois and a current resident of Illinois. All of the medical and dental services must be pre-approved by MAP and will be provided through either inpatient or outpatient care only at AIMMC facilities by their physicians, dentists and medical staff.
If you are interested in applying for MAP assistance, then we welcome learning more about you through our confidential application process. Download the MAP application, print it and complete all of the appropriate information. Pay particular attention to the supporting documentation requested in the application and forward all of them to Ms. Ruthie Rivera, MAP Manager, via email at Ruthie@mfhf.org, via fax to 773-296-5010, or through physical mail to her attention at AIMMC, 836 W. Wellington Ave., CFE Room 189, Chicago, Illinois 60657.

How does one qualify for assistance?
It is important to note that health care services will ONLY be offered at Advocate Illinois Masonic Medical Center located at 836 West Wellington Avenue, Chicago, Illinois 60657. This is with the understanding that the Masonic Assistance Program will be assisting individuals and their families in choosing physicians, helping to negotiate fees and securing financial assistance from any appropriate agencies.
- First, you must be a member in good standing of any Masonic body
- Second, you must be a resident in the State of Illinois
- All services provdied at Advocate Illinois Masonic Medical Center only
